NEW PATIENTS: FIRST VISIT: $350 (typically 45 - 60 mins in duration)

FOLLOW UP VISITS (IN-PERSON or VIRTUAL): $175 (typically 30 mins in duration)

AD-HOC CLINICAL CALL REQUESTED BETWEEN APPT's: $100 (typically 15 min duration)

FORMS & LETTERS: $25/document

PRIOR AUTHORIZATIONS:  1st PA included at no cost; subsequent PA's @ $75/PA

Notes:

- any additional time beyond typical durations will be prorated at $350/hr

- we accept Cash, Checks, Zelle, Credit Card (incl. HSA & FSA cards), Debit Cards, etc.

- all payments are due at time of services.
